Dr. George W. Middleton
George W. Middleton, M.D. graduated with a bachelor of science degree from the University of Utah, Salt Lake City. He received his medical degree from Cornell University Medical College, New York, New York.
Dr. Middleton then went on to earn his master's degree in surgery from the University of Virginia. He served his internship and residency in general surgery at New York Hospital. He also served a residency in urology at the University of Virginia Hospital.
He was the president of the Salt Lake County Unit of American Cancer Society. He also served as chairman of the Legislative Council of the Utah Medical Association. Dr. Middleton previously held positions as trustee on the governing Board of Cottonwood Hospital, and representative of UMA to the Health Cost Management Foundation.
Dr. Middleton is certified by the American Board of Urology. His academic appointments include clinical instructor at the Family and Community Medicine unit at the University of Utah, Salt Lake City.

Dr. Middleton was elected president of the Utah Urological Society. He has been chief surgeon at Cottonwood Hospital and treasurer of the Utah Medical Association. He was also the assistant speaker of the House of Delegates of the Utah Medical Association. Previous postitions held include president-elect of the Utah Medical Association, and president of the Utah Medical Association.

Dr. Anthony W. Middleton
A graduate of Cornell University Medical College (Alpha Omega Alpha) in 1966, Dr. Middleton performed his urology residency at the Massachusetts General Hospital from 1970 to 1974. Previously, he graduated from the University of Utah (Phi Beta Kappa). Dr. Middleton has served in a variety of leadership and service positions in medical organizations and in the community.
A member of the American College of Surgeons, Dr. Middleton is also a member of the American Association of Clinical Urologists, and served as the national president of the AACU in 1992. He continues to serve as editor of the AACU Fax, a semi-monthly fax update on AACU acivities. He also is a delegate to the American Medical Association, representing the American Association of Clinical Urologists in that position. He served as national chairman of the board of UROPAC, a PAC for urologists, from 1992-1997, the first board chairman of that organization.
Dr. Middleton is a member of the Utah Medical Association. He has served as chairman of the Utah Medical Political Action Committee (UMPAC) from 1980 to 1983; as an alternate delegate to the AMA House of Delegates representing the Utah Medical Association; and as president of the Utah Medical Association in 1987-1988. He serves on the board of directors of the Utah Medical Insurance Association (UMIA), the physician-owned medical liability insurance company owned by the Utah Medical Association and was chairman of that board. Honors include the Utah Medical Association Distiguished Service Award in 1993, and he is listed in Marquis Who's Who in America.
Active in the affairs of the Western Section of the American Urological Association (AUA), he has served as chairman of the Health Policy Committee of the Western Section for years, and has in that capacity organized yearly socioeconomic and practice management forums for Western Section meetings. A recent initiative for the Western Section has been an ongoing socioeconomic and practice pattern questionnaire of Western Section members, being refined yearly, to guide both members and leadership in understanding the increasingly turbulent waters of urologic practice. He is president of the Western Section of the AUA in 2000-2001.
Certified by the American Board of Urology
in 1976, Dr. Middleton has represented the Western Section as its representative to the National AUA Health Policy Comittee for years.
Other positions in organized medicine include president of the Utah Urological Association, president of the Salt Lake County Medical Society, and staff president of the Primary Children's Medical Center. He served the Primary Children's Medical Center as a board member of its foundation for several years as well. He also has served as chairman of the Holy Cross Hospital Division of Urology, and currently serves as chairman of the LDS Hospital Division of Urology.
Dr. Middleton is an associate clinical professor of the Division of Urology at the University of Utah. He also has served on the clinical faculty of the Department of Family and Community Medicine at the University of Utah since 1976.
Active in community affairs, Dr. Middleton has served in leadership positions in his church, and has served on the Board of Directors of the Utah Symphony for several years. He was also named a member of the Crime Reparations Board of the State of Utah by Governor Leavitt.
Academically, Dr. Middleton has been author or coauthor of over 32 scientific papers, a book chapter on urologic topics, and has written and published more than 24 extensive articles on socioeconomic issues in medical publications. He has a particular interest in the management of urologic cancers, and has performed over 960 radical prostatectomies. He has given over 130 lectures to various medical groups regarding medical issues.
He has served as chairman of the Medical Advisory Board of the Uroquest Corporation, a high-tech medical device corporation.

Dr. Lisa Stout
Dr. Lisa Stout received her bachelor of arts degree in biology from the University of Utah, Salt Lake City, Utah. She received her medical degree from the George Washington University School of Medicine, Washington, D.C. Dr. Stout completed her post-graduate training at Kaiser Permanente Hospital, Los Angeles, CA.
Dr. Stout has done research in the following fields:
- Stress urinary incontinence
- Clinical studies in bladder hydrodistention
- Effects of DMSO in mast cell degranulation
- Urinary Cytokines IL-1a and Il-1b presence in cystitis
- Identification of lung morphology
