Telephone Calls
Our telephones are answered by friendly, knowledgeable staff during regular business hours. If your question cannot be answered, the nurse or doctor will be consulted, and your call will be returned. If you are calling for a prescription refill, please have the name and telephone number of your pharmacy. In the event of an emergency after hours, please call the office number. The answering service will direct you from there.

Office Visits
For your office visit, please remember to bring:
- Your insurance/medical card
- Required insurance forms
- Referral form (if required by your insurance)
- Name of the referring doctor
- Completed Patient Information Form
- Medical Questionnaire
If you are coming to obtain a second opinion, we must have copies of your records and X-rays prior to the appointment. Please see that they are released to us. Release forms may be obtained from any physician's office.

Payment/Insurance
If you have insurance coverage, please understand that this is an agreement between you and your insurance company. You are responsible for your bill regardless of your insurance coverage. Co-payments are expected at the time of visit. We will file insurance claims for those plans in which we are members. If you do not have insurance coverage, you will be offered a payment plan based on your household budget. We accept Visa and MasterCard. Please call our Billing Office at 614-451-9602 with all questions about insurance coverage and payment.
We accept all major insurance plans including Medicare and Medicaid. The only major plan that we are not a member of is Prudential.
