• Your insurance card
• Physician referral, if your medical insurance requires it
• Medical reports
• List of prescriptions and/or over-the-counter medication, including dose and frequency
• Recent x-rays or relevant records
• Photo ID

Insurance and Payment Information

We accept Medicare, Blue Cross/Blue Shield, Physicians Health Plan, Physicians Care Network, Medicaid, and select others.

Your insurance company may require a co-payment at the time of your appointment. When necessary, our staff will work with patients to set up a payment plan.
